About Fella Health

Fella Health helps middle-aged men live 10 years longer.

We are the telemedicine clinic for bigger guys, worldwide. We use medical weight loss treatment to solve our fellas' most pressing healthcare need, then remain a fella's core healthcare provider for the rest of their life - focused on men's health, longevity, mental health, and primary care.

We will have served 1 million fellas by the end of 2026, and 10 million fellas by the end of 2028.

Our mission is to solve male obesity worldwide, by accelerating the take-up of GLP-1s in middle-aged men. From this, we’ll become the world’s leading men’s health brand.

We’ve grown >150x since Sept 2022, grown >10x since Nov 2023, and are strongly cashflow positive. We have expanded our state coverage to >95% of US population, have started to expand our clinical protocol beyond weight (e.g. testosterone), and are eyeing up international expansion.

The opportunity as Head of Expansion

Fella is building a world-class team.

As Head of Expansion at Fella, your fundamental role is to lay the train tracks so our trains can travel further 🙂

You’ll expand the reach & depth of our offering so we can better serve fellas as their core healthcare provider for the rest of their life: both within and outside the US.

You’ll get us fully integrated with US healthcare insurers so so we can efficiently bill US fellas’ insurance. You’ll research and execute expansion products so we can excellently serve our fellas for a decade. You’ll lead our international expansion so we can serve more fellas. You’ll spin up pharmacy operations so we can become vertically integrated by owning our own medication supply + fulfillment.

Great performance in this role means the tracks are laid out to serve 10 million fellas across the globe by the end of 2028.

If you nail the role, you’ll be a great candidate for our future COO hire.

This is a full-time, fully remote role, and you’ll work async in the timezone of your choice — as long as you’re around until midday Pacific Time for calls as needed.

You’ll report directly to me, Richie :) (our CEO) You’ll also work most closely with our nascent Expansion team: Insurance/Product Lead (Nacho Santos) & Expansion Associate (Joaquin Fernandez).

Key Responsibilities

Lead Medical Expansion — you’ll expand the reach & depth of our offering so we can better serve fellas as their core healthcare provider for the rest of their life: both within and outside the US.

  • Health System Integration: You’ll have everything working so we can efficiently bill US fellas’ insurance for their services. That means ensuring we’re in-network with major insurers, and spinning up our insurance billing function, then ensuring billing is running smoothly.

  • Product Expansion: You’ll research, flesh out, and execute expansion products so we can excellently serve our fellas for a decade.

  • International Expansion: You’ll lead our international expansion beyond the US into the rest of the English-speaking world, then German-speaking & Spanish-speaking countries, then into India & beyond.

  • Pharmacy & Supply Chain: You’ll spin up our pharmacy operations as we become vertically integrated by owning our own medication supply + fulfillment.

Need to Have

  • Experience: you have 4+ years experience in a high-calibre, great-fit role. You do not need healthcare experience. I have a strong preference for people who understand & enjoy the scary speed of a high-growth startup.

  • Systems: you think in and build out systems. In solving a problem you recognize the underlying system, then focus on improving the system itself to fix the true cause of an issue, automating where possible.

  • Learning: you're an autodidact with impressive speed and resourcefulness. You regularly teach yourself the skills you need from scratch.

  • Trenches: you don’t hesitate to get your hands dirty in the trenches. You’re not too pretentious to grind it out.

  • Complexity: you're highly organized and can complete multiple projects concurrently.

  • Mindset: you calmly handle uncertainty and rapidly changing situations.

  • Past: you've likely been pulled into important problems, taken risks, excelled in new areas. You've likely created something from nothing, however small.

Fella Health is the telemedicine clinic for men with obesity, backed by Y Combinator.With Fella, patients meet an obesity specialist to prescribe FDA-approved medications, while they undertake personalized health coaching through our app.

Obesity is the largest chronic disease in the US (>$100bn spent each year) and the traditional solution ("move more, eat less") only works long-term for ~10% of people. But obesity treatment is radically changing due to a new wave of medication options which are 3x more effective than previous options. In addition, only 10% of those using weight management services are men: all programs market to women, not tackling the stigma men feel around this.

Fella's founder is Richie Cartwright (https://www.linkedin.com/in/richard-cartwright). Richiepersonally struggled with binge eating for 6 years. After studying at Cambridge University, he built and sold an AI company working with the UK government. He built a 10k-follower Instagram community around men's binge eating.
